A significant step towards improving health care in Vienna was recently recognized by the “Transfer Care Station Lusthaus” project from the Leopoldstadt care facility. At the WIGEV Awards 2025, the innovative concept took second place in the “Reliably accompanying people” category and impressively shows how medicine can be linked. Loud gesundheitsverbund.at The transfer care station was launched in 2024 to relieve the pressure on the ortho-trauma department at the Donaustadt Clinic.
With 24 beds, the ward offers space for patients whose acute treatment has been completed but who require further care. The project ensures that hospital beds are available for acute medical care sooner and that patients receive seamless follow-up treatment under medical supervision. A dedicated team around Ivana Lazarevic, Marina Jan and many others are responsible for this.
Seamless healthcare collaboration
But how does it work exactly? The introduction of tiered care will close gaps in care and reduce the burden on clinics lazarus.at. Patients who cannot find external short-term care after a stay in the hospital are accommodated in the nursing homes of the Vienna Health Association (WIGEV). These specialize in medical and therapeutic expertise and are available as special hospitals.
The desired goal is an early discharge of the patient to a suitable home or suitable further care. Another plus point: The transfers take place within a few days without any bureaucratic hurdles. The model not only relieves the burden on the clinics, but also ensures that patients receive the best possible care.
Future-oriented planning and quality assurance
The reasons for this innovative approach are diverse. Older patients recovering from complex medical treatments often require more intensive follow-up care. This is where the Austrian Structural Health Plan (ÖSG) comes in, which has been promoting integrative care planning in Austria since 2013. It is used for structured planning of healthcare and is intended to ensure that quality remains assured for patients. The revised version of the ÖSG 2023 sets a clear focus on balanced and easily accessible healthcare. Demographics and epidemiological developments also require continuous adjustment of health care, which further underlines the importance of projects such as transitional care.
